Below is the registration form and a call for abstracts form and a financial disclosure form. Abstracts will only be considered if accompanied by the financial disclosure form. We intend to have oral and poster presentations. If you wish, you can indicate a preference. Abstracts and disclosure form must be submitted via e-mail to the following address: future-lung-abstracts@i-clic.org
A deadline of June 2^nd at midnight CST will be observed.

Registration can take place by web-based application via the CME office website at: http://www.medicine.uiowa.edu/cme and click on upcoming conferences, via fax at 319-335-8327, or via mailing in of the form with appropriate payment.

Study Group List Server
The Hyperpolarized Media MR Study Group, in order to facilitate discussion among the study group members, has established an e-mail list server.  This is a closed list, meaning only members of the Hyperpolarized Media MR Study Group are able to use this service.  If you wish to participate in this membership service, please contact Kristina King of the ISMRM Central Office.
