Cancellation Policy:
  To cancel registration, you must request a refund in writing by Wednesday, 8 February 2006.  Your fee will be refunded, less a US$75 processing charge.  No refunds can be granted for cancellation after this date.

Special Needs of Disabled Persons: We encourage participation by all individuals.  If you have a disability, advance notification of any special needs will help us better accommodate you.  Please notify us of your needs at least two weeks in advance of the program.

Fees are in US dollars and must be paid in US funds.

Register by Mail:  Print the
registration form, complete, and mail ISMRM, P.O. Box 45690, San Francisco, CA  94145-0690 USA.  Make checks payable to ISMRM.

Register by Fax:   Print the
registration form, complete, with credit card information, and fax to +1 (510) 841-2340.

Registrants attending from outside the United States: Please visit the International Visitors Office web site for information on obtaining a visa.
