    This letter must be on institute letterhead, written by your current supervisor or advisor, and must include the following information:

    • Your current degree track (Bachelors, Masters, Ph.D.. or M.D. Candidate or Ph.D. Fellow/Post Doc or M.D. Clinical Trainee/Fellow)
    • Date you received your degree, or expected date to receive your degree
    • Current focus of study or research

Please have your supervisor indicate their email and office phone number within the signature section of the letter.
