Proposal for Reinstatement of an ISMRM ChapterArticle VIII, Section 4.4 – Chapter Tenure
This application will be reviewed by the Chapter Committee and forwarded to the Board of Trustees with a recommendation regarding reinstatement.
Article VIII, Section 4.4, Chapter Tenure of the ISMRM Bylaws:
A Chapter, once established, shall operate for a period of five (5) years, after which it shall be terminated unless a new petition for its continuation is presented to the Board of Trustees in a timely manner and approved by the Board of Trustees. Notwithstanding the foregoing, if a Chapter fails to organize any scientific activity for a period of three (3) years, it shall be terminated, and, unless there are new circumstances, no petition for its reestablishment shall be permitted for a period of at least one (1) year thereafter. The Board of Trustees shall have the right to dissolve a Chapter at any time if the Board of Trustees concludes that dissolution is in the best interests of the Society.

From the Bylaws, Article VIII, Section 4.1:…Chapters shall at all times strive to reflect the multidisciplinary nature of the Society and shall seek to reflect the same intellectual balance as the Society itself. Chapters shall have the same purposes as the Society and operate at all times in conformity with the Society Bylaws and under the direction of, and with the approval of, the Board of Trustees.

Please list the chapter's financial numbers for the last twelve months, including revenue and expenditures, in accordance with the bylaws.
From Bylaws Article VIII, Section 4.2:…The suggested governance must also require that the Chapter report at least annually to the Society Board of Trustees, which report shall include a summary of the Chapter's activities and an accounting of its revenues and expenditures for the year (or other reporting period).
